We are shaping the future of healthcare, making it easier than ever to fit managing your health into your busy schedule. With remote patient monitoring from Novant Health, you can conveniently access safe, expert care from the comfort of your own home, reduce hospital visits and improve your quality of life.

Reimagining care delivery

Remote patient monitoring empowers you to take an active role in your care, setting you up for success now and in the future. We will send you a device kit that you’ll use at home to:

  • Record your vital signs (like blood pressure, heart rate and weight).
  • Report your symptoms.
  • Track when you take your medication.
  • Educate yourself on your condition and treatment through the included resource library.
  • Communicate with your care team as needed.

You will always be connected to your Novant Health clinicians, who will just be a text, phone call or virtual visit away. Your care team will monitor your progress daily and will quickly contact you through your tablet if there’s any concerns with what you report.

How to sign up for remote patient monitoring

The remote patient monitoring program is currently only available to a select group of clinics and providers.

Ask your provider if they are a participating clinic and if you meet the clinical criteria to benefit from the program.

Once you’re enrolled in the remote patient monitoring program, we will:

  • Mail your Health Recovery Solutions PatientConnect™ Complete device kit directly to you.
  • Your included tablet to welcome you to the program, walk you through how to use your equipment, and answer any questions about your individualized care plan.

What’s included in your kit

  • One 4G Samsung tablet with two charging blocks, two USB charging cables and one USB extension cable
  • Bluetooth Peripherals biometric monitoring device; you may receive one or more of these devices to record your vitals:
    • Blood pressure monitor and blood pressure cuff
    • Glucometer with glucose test strips and lancets
    • Pulse oximeter with lanyard
    • Stethoscope with wireless charging dock
    • Thermometer with disposable probe covers and holding case
    • Weight scale
  • Tape for re-packaging
  • Extra batteries

Keep the box your kit comes in, as you’ll use it to send the equipment back later.

KadiraMobile set up guide

  1. Download the Kardia app from the App store or Google play to your personal phone, iPad, or other compatible device.
  2. Launch the Kardia app and select “Create Account.”
  3. Terms of Service & Privacy Policy screen appears. Check the first two boxes to confirm you are 18 years of age or older and that you agree to the Terms. The third box is optional based on if you want to receive marketing emails from the company.
  4. Create your account:
    1. Enter your preferred personal email address.
    2. For your password: Please use your last name with a capital first letter followed by your 4 digit birth year. For example, if your last name is Jones and you were born in 1980, please create your password to be Jones1980. If your last name is over 16 characters, please only use the first 16 letters plus your 4 digit birth year. If your last name is only 3 letters, please add a zero at the end of your birth year so that you will meet the 8- character requirement.
    3. Please leave "United States" in the country field.
  5. Set up your profile: Enter your First name, Last name, and date of birth that matches your medical information on file at Novant Health. When finished, review that the information entered is correct and click “Complete Profile.”
  6. You may receive a prompt to enable Bluetooth or to allow notifications. These are optional and the device will still record regardless of selections.
  7. “Get More with KardiaCare” screen may appear. This is offering you to purchase an upgraded plan for a cost. You do not need to upgrade to Kardiacare to use the device. Select “x” to bypass this prompt.
  8. You will get a “Welcome To Kardia” screen. Select “Connect your Kardia Device.”
  9. Next, you will be asked “Which EKG device do you have?” Please select the option for “KardiaMobile.”
  10. You should get "How to record an EKG" instructions. Take the KardiaMobile device out of the box and set it on a flat surface close to your phone or iPad. Both the device and your phone/iPad should be sitting within a few inches of each other on a flat surface, such as a table or desk, in front of you. Recordings are best taken while seated in a chair while your hands rest on the flat surface. Moisten your fingers if your skin is dry. Hit the next button.
  11. You may receive a prompt to turn on your microphone to record the EKG. If so, please select “Grant Access.”
  12. When your devices are on the flat surface close together and you are ready to begin recording, place two fingers from each hand on the silver squares of the device.
  13. The KardiaMobile device will automatically start recording once your fingers are placed. You should see the application start recording and a 30 second countdown will begin. Please leave your fingers in place for the full time without movement or background noise. If you are alerted that the signal is not strong enough, please move the device close to the speaker part of your phone.
  14. After the time is up, your results page will appear.
    1. Go to the “More Options” section.
    2. Select “DOWNLOAD PDF.”
    3. Enter your password you created for the application (Last name + birth year) and select “Next.”
    4. Another screen may appear saying your document is password protected and if so, enter the password again in the field.
  15. Your EKG recording document will appear with your name, birthday, and timestamp. You should be given a link to share this file. Select the ‘share’ option and locate your email option. Once you select this, it should open a New Message screen with the PDF file already attached.
  16. When you are ready to email your file, please send the file to HVIremotepatientmonitoring@novanthealth.org.
  17. When finished emailing, you can return to the Kardia app and select “done.” You can close the application until your next use.
  18. At the completion of the program, you will be asked to return the KardiaMobile device to the heart and vascular team. If you are scheduled for a follow up visit at the end of the program, please bring the device with you to turn in. This device should not be shipped back with your other devices that you receive from the program such as the tablet, blood pressure monitor, and weight scale.

Turn on your tablet.

Press and hold the single button on the top of the tablet, above “On/Off” printed on the case.

Plug in your tablet.

  1. Plug the prongs of the charging block into an electrical socket.
  2. Place the appropriate end of the charging cable into the available slot in the charging block.
  3. Place the other end of the charging cable into the charging port in the tablet, indicated by the word “Charge” printed on the case.

We recommend keeping your tablet plugged in and on at all times so that you don't miss any alert notifications.

Turn audio alerts on.

Tap on the settings menu (gear icon) in the lower right corner and ensure that the “On” next to “Audio Alerts” is selected/highlighted in yellow.

Answer daily survey questions.

  1. On your tablet screen, select on the time indicated in the “Survey” section. Questions will appear one at a time.
  2. Read the instructions and the question, then select your desired answer.
  3. When the option you selected changes color, select “Record” (yellow button).
  4. If you have additional survey questions, the next one will appear once you have selected “Record.”
  5. When you have answered all survey questions, the survey window will close and there will be a green check mark next to the listed time in the “Survey” section.

Your care team will design your survey questions specifically to help them better understand your health progress.

Record when you take your medications.

  1. Select the appropriate time indicated in the “Take Medicine” section.
  2. Select each medication name that you have taken. Once selected, the surrounding around the medication name will become lighter blue and a green check mark will appear next to the name. The medications icon (pill bottle on the left side of the page) will adjust as you select more medications.
  3. Select “Record” once you have taken the appropriate medications. The window will close. When you have recorded all medications for a given time, there will be a green check mark next to the listed time.

Use Bluetooth devices to record your vitals.

Use each device as instructed when you select either the listed time or “Press Here” (below the heading of the vital you would like to record). If the transmission was successful and you have audio alerts turned on, the tablet will say the readings out loud.

Answer video calls.

When you receive a video call, your tablet will audibly ring (unless you have the volume turned off) and have a message indicating that there is an “Incoming Video Call From the Clinician.” Select “Answer” to connect to your clinician and begin the video visit.

Home-based cardiac rehab

Your physician may have also referred you to home-based cardiac rehab. If so, you will receive additional equipment in your remote patient monitoring kit, such as an in-home EKG device.

Home-based cardiac rehab includes all of the benefits of the remote patient monitoring program, plus:

  • Individualized exercise prescription
  • Education on exercise guidelines and safety
  • Personal coaching by a clinical exercise physiologist

Tablet seems to be having trouble

Most issues can be resolved by restarting your tablet. To restart your tablet, either hold down the power button, or:

  • Go to the settings menu (gear icon).
  • Select “Restart/Shut Down."
  • Select “Restart."

Screen looks different

  • If your screen has a message indicating “Monitoring Paused,” your clinician has paused your enrollment.
  • If your screen is white with the Health Recovery Solutions logo saying “Assign a Patient,” your clinician has ended your enrollment.
  • If either has happened and you are unsure why, please contact your clinician.

How to return your remote patient monitoring equipment

  1. Put all equipment that you received back in the original box it came in.
  2. Seal the box with the provided circle tape.
  3. Keep the box in a safe place indoors.

A mail carrier will come to pick up the box within a couple of days. They will have a return shipping label for the box. Make sure that the new label covers the old label completely before the carrier takes the box.
